Summary:

The Home Support Worker (HSW) provides in-home support, care, and assistance to clients of the Home and Community Care Program. The HSW supports routine activities of daily living to help clients remain as healthy and independent as possible while maintaining dignity, compassion, and cultural sensitivity.

 

The HSW delivers services based on individual care plans and in accordance with relevant program policies and procedures, applicable legislation, accreditation requirements, and Chippewas of the Thames First Nation (COTTFN) policies. The role works in collaboration with the broader health care team and supports the vision of the Chippewa Health Centre and COTTFN.

Responsibilities:

  • Provide home care services based on assessed needs and care plans provided by the Home & Community Care (H&CC) Nurse. 
  • Prepare nutritious, economical meals (including special diets where required). 
  • Perform light housekeeping and assist clients with activities of daily living (e.g., walking, stairs, transfers, getting in/out of bed, eating, dressing, bathing, hygiene). 
  • Operate and support the safe use of mobility devices/equipment (e.g., walkers, wheelchairs, crutches), using safe body mechanics and established guidelines. 
  • Provide care to children during illness/absence of parents and provide assistance to adults requiring support.
  • Observe and report significant changes in client condition or household environment to the H&CC Coordinator/RPN in a timely manner. 
  • Report accidents/incidents and complete required documentation/forms. 
  • Complete shopping for clients when directed, and accompany clients to approved appointments/outings when requested (e.g., medical appointments, shopping, banking, community programs) as approved by the H&CC Nurse. 
  • Support medication routines by ensuring medication is taken as directed and stored properly (per care plan/direction). 
  • Encourage client independence and decision-making; provide assistance only where clients cannot safely do tasks themselves. 
  • Demonstrate knowledge of and respect for clients’ values and lifestyles; maintain professionalism with clients and families.
  • Document daily activities and relevant information in Home and Community Care client charts.
  • Promote community education and awareness of the Home and Community Care Program

Experience Required

Qualifications:

  • Ontario Secondary School Diploma, GED, or equivalent. 
  • Previous experience working with elderly, frail, disabled, or in a related health field is an asset. 
  • Demonstrated understanding of Chippewas of the Thames culture and traditions.
  • Valid driver’s licence, driver’s abstract, proof of insurance, and reliable vehicle. 
  • Clear Vulnerable Sector Check: upon hire and annually (or as requested by the employer); must maintain a clear record for duration of employment. 
  • Current and valid First Aid/CPR certificate. 
  • Current/valid training and certifications as required by Health accreditation/organizational needs (may include: Mental Health First Aid, safe food handling, privacy training, ASIST, safeTALK, diversity training, etc.). 
  • Signed Oath of Confidentiality and Code of Conduct upon hire and annually thereafter. 
  • Maintain current immunization record and TB test record, and complete physical exam to level required for job demands.
