TERM: Full-time, 37.5 hours per week, Monday to Friday, 8:30am – 4:30pm (evenings and weekends may be required)

Summary:

The Diabetes Health Educator plays a vital role in improving the health and well-being of community members of Chippewas of the Thames First Nation through the delivery of diabetes-focused community health education. This position provides community members with the knowledge and tools needed to optimize their health by offering current, research-based education and promoting positive health care practices and lifestyle changes. The Diabetes Health Educator delivers community health instruction, guidance, and support to Chippewas of the Thames First Nation people in areas such as safe practices, proper use of the health care system, and identifying individual and community health needs. The Diabetes Health Educator ensures that the objectives of the Diabetes Program are effectively achieved.

Responsibilities:

• Conduct holistic diabetes assessments and provide ongoing support, education, and monitoring based on shared client goals.
• Use current, evidence-based screening and assessment tools to identify needs, guide interventions, and support early detection.
• Deliver individualized counselling on nutrition, blood sugar monitoring, physical activity, and healthy lifestyle practices.
• Provide education on diabetes prevention, complication reduction, and safe self-management for clients, families, and caregivers.
• Explain diabetes-related medications and lab results (e.g., A1C, glucose patterns, lipids, blood pressure, kidney-related tests) in collaboration with the care team.
• Coordinate monthly Foot Care Clinics with a Chiropodist and provide follow-up education for clients receiving treatment.
• Support school-aged children through screening, diabetes education, and referrals in partnership with school staff.
• Conduct home visits (with approval) to assess needs and ensure supports are in place.
• Plan, deliver, and evaluate diabetes programs (group education, support sessions, workshops) tailored to community needs and priorities.
• Integrate Traditional and Cultural teachings and practices into programming and education initiatives.
• Develop and share educational resources and awareness materials (community events, newsletters, posters, social media).
• Maintain accurate client/program documentation and data entry in required systems (paper/EHR/databases) and support annual work planning.
• Complete required financial and compliance documentation/reporting (P.O.’s, mileage/travel, receipts, funder/internal reports) and assist with proposals as needed.
• Collaborate with internal Health team members and external partners (dietitians, chiropodists, NPs, physicians, hospitals, health units, other First Nations) to support coordinated care.
• Uphold confidentiality and professional standards (PHIPA), maintain required clearances/certifications, and participate in mandatory and role-specific training.
   

Qualifications:

• Certified Diabetes Educator (CDE) designation with the Canadian Diabetes Educator Certification Board (CDECB) required.
• Minimum of a College Diploma or Degree in a relevant health or social sciences field, with current or eligible professional registration as a regulated health care professional (e.g., RN/NP, RD, RPh, MD/DO, PA, or Social Worker) and eligibility for CDE certification.
• At least two (2) years of experience working directly with Indigenous community members, preferably within a community health, wellness, or education setting.
• Indigenous Community Diabetes Support Program certification, Diabetes Education Worker Diploma, or equivalent Indigenous diabetes-specific training is an asset
• Foot Care certification is a strong asset.
• Membership in the Diabetes Educators Section of the Canadian Diabetes Association is an asset.
• Knowledge and understanding of the Chippewa of the Thames First Nation community, culture, and traditions.
• Current certifications (or willingness to obtain and maintain):

  • Applied Suicide Intervention Skills Training (ASIST)
  • Cultural Diversity Training
  • Safe Food Handling
  • Privacy Training
• Current Vulnerable Sector Police Check, maintained in good standing throughout employment.
• Valid driver’s license, driver’s abstract, proof of insurance, and access to a reliable vehicle.
• Flexibility to work evenings and weekends as required.
• Demonstrated ability to serve as a positive role model within the community.
