Job Description

JOB SUMMARY:

The Diabetes Health Educator supports the health and wellness of community members of Chippewas of the Thames First Nation through the delivery of culturally relevant diabetes education and prevention programming. This role provides community members with the knowledge, tools, and confidence to manage or prevent diabetes through holistic, evidence-based education and lifestyle support. The Diabetes Health Educator ensures that the objectives of the Diabetes Program are met by providing high-quality, community-focused care and promoting positive health practices.

 

RESPONSIBILITIES:

Diabetes Education

  • Conducts holistic health assessments, supports, and monitors clients living with diabetes.
  • Provides one-on-one and group education on nutrition, blood sugar monitoring, and physical activity.
  • Screens, educates, and refers school-aged children and community members for early detection and diabetes management.
  • Coordinates monthly Foot Care Clinics and ensures client follow-up.
  • Develops and distributes educational materials (newsletters, social media, displays).
  • Conducts home visits (as approved) to assess and support client needs.
  • Researches diabetes trends and identifies service gaps.

Programming

  • Plans, delivers, and evaluates diabetes education programs, support groups, and community initiatives.
  • Provides presentations on healthy eating, water intake, and traditional wellness teachings.
  • Facilitates healthy food and cooking classes in collaboration with the Dietitian.
  • Hosts monthly diabetes education sessions and integrates cultural teachings into all programs.
  • Monitors program effectiveness and implements improvements as needed.

Administration & Collaboration

  • Maintains accurate records and submits required reports and financial documentation.
  • Works closely with Community Health Nurses, the Health Promotions Worker, and other health professionals to deliver coordinated programs.
  • Networks with schools, local health units, and service providers to enhance education and prevention strategies.
  • Participates in professional development and maintains current certifications.

Education

Post Secondary Diploma

Experience Required

  1. Certified Diabetes Educator (CDE) designation with the Canadian Diabetes Educator Certification Board (CDECB) required.
  2. College Diploma or higher in Nursing, Nutrition, Health Promotion, Community Health, Indigenous Community Care, or a related discipline.
  3. Minimum two (2) years’ experience working with Indigenous communities in health, wellness, or education settings.
  4. Foot Care and Indigenous Diabetes Support Program certifications are strong assets.
  5. Knowledge of Chippewas of the Thames First Nation culture and traditions.
  6. Proficient in Microsoft Office, Electronic Medical Records (EMRs), and social media for health promotion.
  7. Current certifications (or willingness to obtain):
    • First Aid/CPR
    • WHMIS
    • Mental Health First Aid
    • SafeTALK
    • ASIST
    • Safe Food Handling
    • Privacy and Cultural Safety Training
  8. Valid driver’s license, insurance, reliable vehicle, and current Vulnerable Sector Police Check.
  9. Flexibility to work evenings and weekends as needed.
  10. Preference given to First Nation applicants as per Section 16(1) of the Human Rights Act.
