Job Description

Babcock Community Care Centre, located in Wardsville, Ontario is a family owned and operated 60-bed long-term care home dedicated to providing quality care to the residents of the four counties area.

Reporting to the Administrator and Director of Care, the Nurse Practitioner position is a critical role with overall accountability for the management and leadership for health care and services for our residents.

The NP role includes:

  • Ensuring the provision of care is aligned to the needs of each resident, the home’s strategic plan, mission and values.
  • Promoting a resident-centered care approach that engages residents, family members and employees to provide care that meets the individual needs of our residents.
  • Completing daily clinical rounds, orders, evaluating diagnostic tests for residents to identity and to assess resident’s clinical concerns and health care needs.
  • Recording findings and developing action plans in collaboration with the other interdisciplinary team members.
  • Prescribing or recommending drugs or other forms of treatment to improve resident care.
  • Providing leadership and direction in collaboration with internal and external stakeholders for all activities related to nursing.
  • Evaluating clinical outcomes and working with other interdisciplinary team members to continuously improve these outcomes.
  • Ensuring compliance with all relevant legislation including but not limited to: Ministry Acts, regulations, policies and procedures, directives, and collective agreements.
  • Collaborating with the Medical Director, other physicians and community services/agencies in the provision of quality health care for the resident.
  • Conducting physical examinations and preventative health measures within prescribed guidelines and the standards of the provincial College of Nurses.
  • Providing assessments for residents with High Intensity Needs (HIN).
  • In collaboration with the Director of Care, providing information for Critical Incidents, as needed.
  • Assisting with the onboarding of new hires and education of staff, as required.
  • Mentoring nursing staff with respect to nursing practices.
  • Actively participating in conferences, workshops and other educational opportunities for further development of best practices and nursing knowledge.
  • Working in accordance with the organization's Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act.
  • Performing other duties as assigned.

Education

Post Secondary Degree

Experience Required

Qualifications:

  • Current registration as a Nurse Practitioner, in good standing with the provincial College of Nurses.

  • Must have a minimum of one (1) year’s working experience as a Nurse Practitioner in a healthcare setting, preferably long-term care.

  • Strong relationship building abilities and organizing skills with the ability to lead and influence others.

  • Must posses excellent written and oral communication skills.

  • Demonstrates knowledge of current trends and best practices relative to nursing.

  • Proficiency with Microsoft Office, Point Click Care and other computerized documentation systems.

  • Must possess current CPR certification.

  • Certificate in Gerontology and current membership with the GNA would be considered an asset.

  • All applicants must successfully pass the prescribed Vulnerable Sector check.

  • All applicants must provide proof of being fully vaccinated, except due to legitimate established exceptions.

  • Have a passion to promote person-centered care and work with the senior population.
