Job Description
Position Summary
The Direct Support Professional-AWAKE NIGHTS provides supports during the night and early mornings to ensure the health, personal welfare, and safety of individuals who access supports. It is recognized that this position may sleep during a portion of the night, however, is expected to wake and take appropriate actions as required.
Specific Duties and Responsibilities
Person Directed Planning:
Understands, translates into actions and assignments as directed and implements the person’s personal outcomes measures (POMS) for individuals accessing supports as prescribed.
Safety, Health and Well Being of the Person:
Implements prescribed health, safety, medical and required strategies and interventions for designated individuals ensuring the accuracy and completeness of prescribed procedures and documentation.
Ensures that at the beginning of each shift that the Direct Support Professional-AWAKE NIGHTS is aware of any medication changes or communication updates
Personal Care:
Provides personal care and support for the person accessing supports as prescribed, including but not limited to, waking, sleeping, bathing, hygiene, dressing, a positive personal appearance, eating, transferring, in a way that facilitates each person’s competence, respect and dignity through the normative routines and rhythms of the day, week and month.
Activities of Daily Living:
Provides direct assistance and support based on the person’s support plan that enables each person supported to carry out the required activities of daily living including but not limited to, personal care and hygiene, medication.
Home Maintenance:
Through direct action as prescribed for the individual accessing supports, maintains the internal and external appearance of people’s homes to normative community standards. Inspects monitors and ensures all safety aspects are in place for safe overnight operation e.g., hallways clear, inspections done.
Administration and Records:
Inputs, updates and maintains all prescribed records and files, including but not limited to information pertaining to people accessing supports, requirements as an employee and communication with the agency.
Internal/External relationships:
Maintains and improves relationships with internal team members. Maintains a good working relationship with all internal/external roles such as families. Monitoring of other locations, as required.
General:
Embrace and promote an individualized support model and agency philosophy.
Adhere to all agency policies and guidelines,
Participate in training, as required.
Attend mandatory training, as required.
Participate in personal continuous performance development.
Education
Experience Required
Skills
Work effectively with individuals, other staff, families and the community.
Be flexible to change at any time as each person directs his or her choice of activities.
Utilize diplomacy and tact in all interactions with others, maintaining a professional demeanor always.
Ability to work independently.
Demonstrated ability to clearly transmit and receive information by email, phone and in person.
Solid interpersonal and teamwork skills.
Ability to be creative and problem solve independently in challenging situations, as required.
Basic computer skills and ability to record in data base and do online training.
Proven ability to adapt and respond to various styles of communication from other team members and individuals accessing services.
Be flexible to work with additional teams or individuals, as needed.
Qualifications
Post-Secondary Education in a related field or experience in a related field.
Current First Aid/CPR
Non-Violent Crisis Intervention or willingness to get
Clear Vulnerable Sector Police Check
Valid driver’s license (G2 or G) and use of a reliable vehicle
Liability Insurance, minimum of $2,000,000
